Requirements for Birth Certificate Civil Registration

  • Presence of the person to be registered must accompany the informant.
  • Original of the Live Birth Certificate or Civil Registration of the person to be registered. The certificate or registration must be issued where the birth occurred. If you do not have a live birth certificate, you must have sworn statements by two witnesses who are aware of or were present at the birth.
  • If the baby is more than one month old, one of the above documents or the baptism certificate must be presented.
  • Identification of the parents: if they are Colombian, photocopies of their original citizenship cards on separate sheets of paper. If either of the parents is of a different nationality, his/her valid passport and a copy of same must be presented.
  • Certificate of blood type and RH factor of the person to be registered.
  • Please note that documents issued by another state in a language other than Spanish must be translated by an official translator and have an apostille or be legalized, as the case may be.
